- PSA-issued birth certificate or the identity document specifically requested

- Correct full name, address, citizenship, and guardian information
Academic records
- Latest report card, Form 138/SF9, or certified true copy of grades
- Transcript or certificate of grades when required
- Enrollment, admission, or graduation certification when applicable
- Good Moral Character certificate if listed by the provider
Financial and household records
- Latest parent or guardian Income Tax Return, if applicable
- BIR Certificate of Tax Exemption or Non-Filer certification
- Barangay/DSWD indigency certificate or social case study when accepted
- OFW or seafarer contract/proof of income where applicable
Program-specific materials
- Completed official application form and privacy consent
- Essay or personal statement within the requested word limit
- Recommendation letters requested early from appropriate referees
- Special-group documentation, portfolio, or proof of awards only when requested
Before uploading
- Scan every page clearly, upright, complete, and readable
- Use the required file type and stay within size limits
- Name files clearly and keep an unchanged backup
- Confirm every required item is attached before final submission
